Nurse / Advanced Practice Clinician / Primary Care Specialist

Job SummaryWe are seeking an experienced nurse or ECP to join our nursing team, providing holistic care to patients in their own homes and in care settings. This role offers a unique opportunity to work in a dynamic and supportive team, developing skills and experience in a variety of settings.
Main Responsibilities- Manage a caseload of housebound patients, providing expert advice and assistance with their ongoing medical needs.
- Undertake acute visits on a daily basis, providing comprehensive care for patients from initial history taking, clinical assessment, and diagnosis.
- Liaise with patients' GPs, as required, and implement treatment plans in collaboration with the practice team.
- Demonstrate safe, clinical decision-making and provide holistic care to patients.

RequirementsEssential Qualifications and Experience- NMC or HCPC registration.
- Knowledge of the needs of patients with long-term conditions.
- Excellent communication skills.
- Ability to work collaboratively with the practice team to meet the needs of patients.
- Independent Prescribing qualification.
- Post-registration qualification in long-term conditions management.
- MSc in Health & Social Care.
