Key Responsibilities:

  1. Work on a rotational shift basis within the Brent Rapid Response Team, providing expert clinical care to patients with undifferentiated, undiagnosed, complex health needs and long-term conditions.
  2. Develop and implement comprehensive short-term care plans to optimise health and wellbeing, preventing hospital attendance/admission or allowing early discharge home.
  3. Monitor standards and performance of the team, ensuring safe and effective clinical service delivery.
  4. Be responsible for your own professional development through active learning and reflective practice.
