Support Worker Job Description

Equipped 4 life 16+ is seeking an experienced Support Worker to join our team. We provide 24/7 support to young people aged 16+ in a residential setting.

Main Responsibilities:

  1. Provide support in a professional, positive, and homely environment, promoting integration and meeting individual needs.
  2. Work independently, follow lone working policies, and participate in training to enhance skills.
  3. Establish positive relationships with young people, assess their needs, and collaborate in producing individual support plans.
  4. Enhance relationships, self-confidence, social skills, and life skills through individual and team work.
  5. Organise and participate in activities for young people, promoting a positive and stimulating environment.
  6. Adhere to reporting and recording procedures, ensuring timely reports and correspondence to a professional level.
  7. Communicate effectively with agencies and professionals, preparing written reports as required.
  8. Act as a professional corporate parent, providing guidance and support while maintaining professional boundaries.
  9. Ensure key working sessions and significant conversations are carried out in a timely and appropriate manner.
  10. Complete daily tasks, including observations logs, log book entries, chronologies, and other documentation as assigned.
  11. Manage shifts as required, ensuring necessary liaison and communication with families, agencies, and the local community.
  12. Act immediately upon any safeguarding concerns, following Equipped 4 life 16+ policies and procedures.

Requirements:

  • 1 year experience as a support worker in a residential or semi-independent provision.
  • Desirable: Health and Social Care NVQ.
