Job Title: Advanced Clinical Practitioner Nurse/Paramedic

We are seeking an enthusiastic, motivated and committed Advanced Clinical Practitioner (Nurse or Paramedic) to join our vibrant and welcoming Primary Care Network.

This is a unique opportunity to work autonomously and be accountable for own professional actions, undertaking clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.

About the Role
  • Work with multidisciplinary teams of ACPs, Clinical Pharmacists, nurses, care coordinators and social prescribers.
  • Manage daily surgeries face to face, online and telephone consultations to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions.
  • Initiate direct referrals as appropriate and arrange hospital admission for suitable cases.
  • Maintain accurate record keeping in SystmOne/EMIS using templates and READ codes.
Person Specification
  • First Level Nurse (RGN)/Qualified Paramedic qualification.
  • Post Graduate diploma or degree in Advanced Clinical Practice.
  • Independent non-medical prescriber qualification.
Salary and Benefits

The salary for this role is estimated to be around £60,000 per annum, depending on experience. We also offer a competitive benefits package, including a salary sacrifice car leasing scheme.
