About the Role

This permanent position is part of the Home First Team, which supports early discharge from hospital by providing care and support to patients in their own homes.

As a member of this team, your duties will include:

  • Supporting patients with personal care and completion of activities of daily living
  • Providing therapy rehabilitation to improve patient outcomes

To be successful in this role, you must have:

  • Numeracy & Literacy Level 2
  • Care certificate (willing to work towards if already holds NVQ level 3)
  • Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; - Theoretical knowledge - Experience of a range of care and related procedures - clinical observations - knowledge of relevant legislation

Additional requirements include completion of the WASP competencies and Trust face to face training session related to insulin administration, and/or oral medication administration, following the Trust approved standard operating procedure and policy.

Main Duties of the Job

As a senior integrated support assistant, you will be responsible for providing both rehabilitation and nursing care following a person-centred plan within the patients' own home. Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post.

Qualifications

Essential

  • Numeracy and Literacy Level 2 (GCSE A-C)

Salary: £18,720-£20,350 per annum (dependent on experience)
