Role Overview
We are seeking a skilled Care and Support Partner to join our team. The successful candidate will be responsible for providing compassionate and personalized support to our clients, helping them to maintain their independence and quality of life.
Key Responsibilities
- Provide individualized care and support to clients, including assistance with personal care, medication management, and household tasks.
- Develop and maintain positive relationships with clients, their families, and other stakeholders.
- Contribute to the development and implementation of client care plans, ensuring that they are tailored to meet the individual needs and preferences of each client.
- Participate in regular team meetings and training sessions to ensure that you are up-to-date with the latest best practices and policies.
