Care Coordinator for Extra Care Services

Creative Support is a not-for-profit organisation that provides high-quality, person-centred support to individuals with diverse care needs. We are seeking a warm, compassionate, and proactive Manager to provide person-centered care and support to our service users.

You will be based in the Extra Care Services in Carlisle and will assist the Extra Care Registered Manager and Deputy Manager in the operational management of the service. You will collaborate with the Extra Care Registered Manager, Deputy Manager, the housing provider, NHS professionals, Social Services, and other stakeholders to ensure that the highest standards of care are provided.

As a Care Coordinator, you will ensure that the care provided empowers clients and meets their needs. You will work in conjunction with service users' families and friends to promote a positive atmosphere within the service. The ability to work in an empathetic and holistic way in order to maintain orientation and independence is essential.

We value the personal skills and interests you will bring to both the role and to the lives of the people we support. Previous experience in services for older people, people with dementia, and memory loss is essential. Your approach will be warm, person-centered, and respectful.

Creative Support is committed to your continuous professional development and you will have the opportunity to explore your training and development needs. Full training is provided, as is the opportunity to work towards QCF Diploma in Health and Social Care.
