You will be part of a multidisciplinary team, working collaboratively with other professionals to deliver high-quality care and support.

Key Responsibilities
  • Deliver specialist assessments, reviews, and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS).
  • Contribute to the development and implementation of care plans, working collaboratively with other professionals to achieve the best possible outcomes for our patients.
  • Liaise with other professionals and organizations to ensure integrated and holistic care.
  • Participate in local audit and research projects, broadening research and development skills.
