Key Responsibilities
This role involves:
- Working closely with clients, their families, and care teams to coordinate care and rehabilitation
- Developing and implementing tailored care plans
- Maintaining regular contact with clients and their families to monitor progress and address any concerns
Salary and Benefits
As a valued member of our team, you will receive a competitive salary of £40,000 - £48,000 per annum, depending on experience. You will also benefit from:
