Job Overview

Epsom and St Helier University Hospitals NHS Trust is seeking an experienced Clinical Pharmacist to join their team. The successful candidate will be responsible for providing a coordinated pharmaceutical service, promoting safe, legal, and economic use of medicines, and ensuring seamless transfer of pharmaceutical care for patients.

Main Duties
  1. Participate in the ward/clinical pharmacy service to monitor prescriptions and provide pharmaceutical advice/drug information to healthcare professionals and patients/carers.
  2. Identify potential risks to patients when screening prescriptions and resolve relevant issues prior to dispensing.
  3. Ensure seamless transfer of pharmaceutical care for patients by liaising with other acute Trusts, GPs, Community, and PCT pharmacists as appropriate.
  4. Counsel patients/carers regarding their medication.
  5. Encourage support and compliance with the Trust formulary and local prescribing guidelines.
  6. Undertake in-patient, discharge, and out-patient dispensing and checking using the Pharmacy Computer System and in accordance with departmental policies and procedures.
  7. Participate in the dispensing and checking for clinical trials and controlled drugs, completing relevant documentation in accordance with departmental procedures as required.

To work with the pharmacy team to deliver a service that meets the pharmaceutical needs of patients within the Trust and other external organisations to whom a service is provided.

Detailed Job Description and Main Responsibilities
  1. Work within agreed Staffbank and departmental policies and procedures and accepted standards of practice.
  2. Work on other sites in the Trust by agreement.
  3. Undertake such other duties as may be required from time to time, as are consistent with the responsibilities of the grade.
  4. Ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity.
  5. Treat patients and customers with dignity and courtesy.
  6. Ensure that medicines do not fall into unauthorised hands.
  7. Have responsibility for the health, safety, and welfare for self and others and to comply with the requirements of the Health and Safety regulations, including COSHH.
  8. Promote Equal Opportunities and to comply with the requirements of the legislation and the Trust and StaffBank policies.
  9. Attend such training courses and meetings as are necessary for the efficient performance of the post holder's duties.
  10. Participate in Continuing Professional Development (CPD), maintaining own CPD portfolio.

  12. Comply with the Trust's No Smoking Policy.
  13. Comply with Duty of Candour principles.
Person SpecificationQualifications/Education
  1. Registered Pharmacist with General Pharmaceutical Council.
  2. Pharmacy degree – MPharm Pre-registration training.
  3. Diploma in Clinical Pharmacy or equivalent.
Experience
  1. Post registration UK hospital experience.
  2. Demonstrate evidence of excellent clinical knowledge and skills.
  3. Experience of providing services to a broad variety of patients.
  4. Medicines Information.
