The Hospice of Windsor and Essex County is seeking a compassionate individual to fill the role of Social Worker. This position involves providing psychosocial assessments, developing goals and related plans of care in collaboration with patients and caregivers, and making recommendations regarding appropriate social work interventions.

Job Responsibilities:
  • Complete comprehensive psychosocial assessments of patients and their caregivers;
  • Develop and implement individualized plans of care in collaboration with patients and healthcare teams;
  • Maintain accurate records of patient interactions, assessments, and progress notes;
  • Collaborate with interdisciplinary teams to ensure seamless delivery of palliative care services;
  • Provide emotional support and counseling to patients and their families as needed.

We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience.
