Job Title: Diabetes Nurse Specialist

We are seeking an experienced Diabetes Nurse Specialist to join our team at West Leeds Primary Care Network (WLPCN). The successful candidate will work closely with our multidisciplinary team to deliver high-quality diabetes care to our patients.

About the Role

The Diabetes Nurse Specialist will be responsible for the assessment, planning, and implementation of clinical management plans for patients with diabetes. This will include providing personalized care and support to patients, as well as educating healthcare professionals across the pathway.

The successful candidate will work closely with our Pharmacy Team, Occupational Therapists, and other healthcare professionals to develop and deliver a comprehensive diabetes service. This will include supporting the management team in reviewing clinical policy and procedure, and working closely with secondary care to provide seamless transition for patients.

Key Responsibilities
  1. Assess and plan clinical management plans for patients with diabetes.
  2. Provide personalized care and support to patients, including education and lifestyle advice.
  3. Work closely with healthcare professionals across the pathway to develop and deliver a comprehensive diabetes service.
  4. Support the management team in reviewing clinical policy and procedure.
  5. Work closely with secondary care to provide seamless transition for patients.
Person Specification
  • Registered Nurse with active NMC registration.
  • Independent prescriber with experience in GLP1 & insulin initiation.
  • Able to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
