Support Worker

About the Role:
The successful candidate will be responsible for providing high quality support to individuals with learning disabilities, enabling them to live independently and participate fully in their communities. The role involves working collaboratively with service users, their families and other professionals to develop and implement person-centred care plans.
Key Responsibilities:
• Provide day-to-day care and support to individuals with learning disabilities, including personal care, housekeeping, and meal preparation.
• Assist individuals with their personal goals and aspirations, promoting independence and empowerment.
• Work collaboratively with service users, their families and other professionals to develop and implement person-centred care plans.
• Ensure that all services are delivered in accordance with recognised standards, including CQC and Supporting People standards.
• Participate in the induction of new colleagues and participate in the service user reviews with line manager or other colleagues.
Requirements:
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with service users, their families and other professionals.
• Ability to maintain confidentiality and handle sensitive information.
• Ability to work flexibly and as part of a team.
