About the Role
Havering Health Ltd is seeking a highly skilled Pharmacy Technician to join our team in a clinical support role. As a Pharmacy Technician, you will play a vital part in improving the quality of prescribing and patient outcomes across our network through high-quality medicines review and optimisation.
Key Responsibilities
- Work closely with practice teams, pharmacy colleagues, and patients to ensure coordinated and efficient processing of prescription requests, synchronising medication, setting up electronic repeat dispensing, and monitoring medicines use to prevent waste or over-usage.
- Carry out medicine reconciliations for discharge summaries, clinic letters, and other transfer of care communications.
- Work with clinicians and the pharmacy team to develop and implement robust systems for shared care prescribing and high-risk drug monitoring and other drug monitoring for optimisation of long-term conditions.
- Update and maintain accurate and comprehensive patient medication records.
Improving Patient Safety, Audits, and Supporting Reviews
- Work with the pharmacy team and clinicians to deliver QIPP/PIS and other local commissioned enhanced services to improve medicines optimisation.
- Work alongside the pharmacy team to monitor and action safety and drug alerts in a timely and appropriate manner, including liaising with patients when necessary.
- Work with clinicians and pharmacy team to undertake medicine audits and other quality improvement/project work as required.
- Work alongside the admin team to promote and develop high-quality prescribing processes, including identifying quality improvement opportunities.
Working in a Wider Team (In Practice)
- Develop positive and professional working relationships with colleagues to ensure excellent patient care.
- Promote best practice for medicines use, including supporting training of practice teams on medicines processes and policies.
- Respond to medication queries or requests for medicines information from patients and staff in a professional and timely manner, including escalating to clinicians or senior pharmacy colleagues when necessary.
- Work in practice with practice teams to support the achievement of prescribing outcomes for Quality Outcomes Framework.
- Undertake any other additional duties appropriate to the post as requested by the Partners or PCN Clinical Directors or Senior Clinical Pharmacist.
Person Specification
Qualifications
- Qualified Pharmacy Technician.
- Current registration with the General Pharmaceutical Council (GPhC).
Experience
- Evidence of continued professional development (CPD).
- Experience of working as a qualified, registered technician in primary care, community, or hospital pharmacy.
- Understanding of the aims of current healthcare policy and appreciation of the implications of this for clinical pharmacy in PCNs.
- Knowledge of the principles of medicines optimisation.
- Knowledge and understanding of pharmacy law and ethics and current legislation.
- Good clinical pharmacy knowledge, including terminology.
- Experience of providing pharmaceutical and prescribing advice.
- Experience of clinical audits.
- Experience of delivering education and training.
- Ability to analyse and interpret prescribing data.
