About the Role:

We are seeking a highly skilled and compassionate Care Leaver Support Specialist to join our team at Diamond Blaque HR Solutions. As a key member of our staff, you will play a vital role in supporting care leavers as they transition to adulthood.

Key Responsibilities:

- Provide coaching and motivational support to enable young people to build confidence and self-esteem.
- Help care leavers develop practical independent living skills, including managing finances and holding a tenancy.
- Ensure young people are effectively engaged in suitable accommodation, education, training, or employment appropriate to their abilities and aspirations.
- Contribute to the assessments, formulation, implementation, and review of Pathway Plans through direct interventions and full engagement of the young person.

Requirements:

- Degree level or extensive experience directly working with children and young people.
- Advanced Microsoft Office skills, including Word, Excel, and Outlook.
- Experience working with vulnerable children, young adults, and families according to statutory guidance directly in or in partnership with local authorities' children's services.
- Enhanced DBS Checks are required for the role.
