About the RoleWe are seeking a highly motivated and compassionate Support Worker to join our team at Home Group. As a Support Worker, you will play a vital role in supporting individuals with complex needs, including autism and learning disabilities, to live fulfilling and meaningful lives.
Key Responsibilities- Provide person-centred support to individuals with complex needs, promoting independence and social inclusion.
- Work collaboratively with the clinical team to develop and implement support plans that meet the individual's needs.
- Monitor and report any changes in the individual's mental and physical health, ensuring their safety and wellbeing.
- Support individuals to participate in activities that promote their interests and well-being.
- Work on a rota basis, including days, evenings, weekends, and bank holidays.
- Experience in a complex care setting, with a Level 2 Diploma in Adult Health and Social Care or willingness to work towards it.
- Ability to work with individuals with complex needs, including autism and learning disabilities.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- Enhanced DBS check required.
