Job Overview

This is a challenging and rewarding opportunity to join our team as a Registered Nurse (RN) Case Manager. As a key member of our Interdisciplinary Group, you will be responsible for assessing and identifying patient/family needs, coordinating the Plan of Care, and providing palliative and supportive care.

Salary & Benefits

We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience. In addition, we provide a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Job Responsibilities
  • Assess and identify patient/family needs, utilizing the nursing process to develop and implement individualized Plans of Care.
  • Coordinate with the Interdisciplinary Group to ensure seamless delivery of care and services.
  • Provide education and support to patients, families, caregivers, and other healthcare professionals about disease management, palliative interventions, and end-of-life care.
  • Participate in Quality Assessment and Performance Improvement activities to continuously improve our services and outcomes.
Requirements & Qualifications
  • CURRENT RN license in the state where the employee will be working.
  • MINIMUM one (1) year of in-patient nursing or hospice experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate.
  • Previous experience working with an Electronic Medical Record (EMR) system.
  • Valid driver's license and automobile insurance per Company policy.
Competencies & Skills
  • Satisfactorily complete competency requirements for this position.
  • Ability to effectively communicate in English (verbal and written).
  • Familiarity with electronic media and ability to advance skills.
