Community Care Specialist

We are seeking a highly skilled Community Care Specialist to join our team in North Liverpool. As a key member of our district nursing team, you will play a vital role in delivering high-quality patient care and contributing to the development of our integrated community care service.
Key Responsibilities- Deliver clinical care to a defined population, working effectively with senior practitioners and other healthcare professionals to meet patient needs.
- Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations.
- Promote and deliver evidence-based care, adhering to national and local guidelines.
- Assess and manage treatment plans for patients at risk, working collaboratively with the team to ensure seamless care delivery.
- Produce accurate and complete records of patient consultations, consistent with legislation, policies, and procedures.
- Registered Practitioner with relevant clinical experience in a similar field.
- Mentor/teaching qualification or equivalent work-based experience.
- Evidence of clinical supervision experience.
As a Community Care Specialist with our trust, you can expect a competitive salary of £29,970 - £36,483 per annum, depending on your experience.
