Lead Therapy in a Frailty Setting

Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust is seeking a highly skilled and motivated Highly Specialist Physiotherapist or Occupational Therapist to join our dedicated team within the Frailty SDEC unit. This role offers an exciting opportunity to make a real difference in the lives of elderly patients.

Your Responsibilities:

  • Provide expert clinical care, focusing on safe discharge planning for elderly patients back into their communities.
  • Manage a complex caseload of patients, ensuring high-quality and evidence-based treatment plans are implemented.
  • Collaborate effectively with a multidisciplinary team to deliver holistic patient care.
  • Contribute to the development and implementation of innovative therapy programs within the unit.
  • Mentor and support junior therapists, fostering a culture of continuous learning and professional growth.

About You:

You are a registered therapist with extensive experience in working with elderly patients. You possess strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care. Your leadership qualities and commitment to teamwork will be invaluable assets to our team.

Essential Requirements:

  • Active registration with the HCPC as a Physiotherapist or Occupational Therapist.
  • Proven experience in rehabilitation and chronic disease management, particularly within an elderly population.
  • Demonstrated ability to manage complex caseloads effectively.
  • Strong interpersonal and communication skills, enabling effective collaboration with patients, families, and colleagues.
