Job Title: Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at the NHS. The successful candidate will be responsible for providing high-quality patient care, working autonomously within their professional scope of practice, and collaborating with the multi-disciplinary team to deliver safe and effective care.

Main Responsibilities:
  • Provide clinical leadership and expertise in the management of long-term conditions, including asthma, COPD, diabetes, and coronary heart disease.
  • Work collaboratively with the team to develop and implement care plans, ensuring patients receive comprehensive and coordinated care.
  • Conduct clinical assessments, diagnose, and manage acute and chronic conditions, using advanced autonomous clinical skills and a broad in-depth theoretical knowledge base.
  • Prescribe and review medication, ensuring it is appropriate to patient needs and in accordance with latest evidence-based practice, national, and practice protocols.
  • Develop and maintain a Personal Learning Plan, ensuring up-to-date evidence-based knowledge and competence in all aspects of the role.
Requirements:
  • Minimum of 5 years post-registration experience as a Registered General Nurse.
  • Postgraduate qualification at level 7 or above in a specialist area.
  • Teaching or mentoring experience and/or qualification.
  • Advanced Digital badge - obtained through an accredited programme or the e-portfolio supported route.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to analyze data and information, drawing out implications for the patient/impact on care plan.
