Cardiovascular Disease Specialist Nurse

County Durham and Darlington NHS Foundation Trust is a leading integrated care provider in England, serving a population of around 600,000 people. As a key member of our team, you will be working core hours of 09:00-17:00 Monday-Friday with currently no weekend working.
This exciting role involves delivering cardiac rehabilitation to patients with CHD and HF, alongside exercise facilitators. You will have the opportunity to develop your clinical skills through training and gain certifications such as NMP.
Job ResponsibilitiesThe successful candidate will provide nursing care, support, and monitoring to patients with cardiovascular disease and heart failure. This will involve:
- Delivering cardiac rehabilitation both face-to-face in classes and home-based (heart manuals and/or digital)
- Supporting patients to optimise their treatments prescribed following diagnosis and monitoring effects
- Education and support around lifestyle issues and risk reduction
You will also have the opportunity to work independently and travel around the Durham/Derwentside area, with possible support for other teams across the CDDFT footprint.
Person SpecificationStatutory Registration- Registered health professional registered with the NMC
- Able to travel independently around County Durham & Darlington
- Flexible attitude to work
- Accredited cardiology course or willingness to work towards
- Knowledge/experience of cardiac rehabilitation
- Knowledge of national and regional policies influencing cardiovascular disease
- Post-registration experience of working with cardiac patients
- Experience of working in a community setting
- Understanding common cardiovascular pathophysiology and cardiac conditions
- Ability to accurately record clinical data and understand their significance
- Phlebotomy or willingness to work towards
- Evidence of team working and care planning
- Ability to work independently and with minimal supervision
- Understanding use of health-related questionnaires and patient-reported outcome measures
- Participation in clinical supervision
