About the Role

We are recruiting for an experienced and compassionate Community Mental Health Support Worker to join our Crisis Resolution Home Treatment Team at Southern Health NHS Foundation Trust.

Job Summary

This is an exciting opportunity for a skilled professional to work with individuals in crisis, providing support and interventions to help them recover and live independently in their community.

Main Responsibilities
  • Organise and participate in defined assessment methods/activities to identify and address service users' health and wellbeing needs
  • Arrange care review meetings in line with Trust policy to ensure continuity of care
  • Develop and review appropriate care plans, identifying and organising additional resources as necessary
  • Contribute to the administration of relevant paper and electronic documentation in accordance with Trust Record Keeping Policies
About You

To be successful in this role, you will have:

  • A foundation degree or NVQ in Health and Social Care level 3, or be working towards completion within the next three months; alternatively, you will have high-level experience in a similar role
  • Proven knowledge and demonstrable skills in caring for people with mental ill health
  • An understanding of issues relating to the conditions of mental illness
  • Experience of working with mental ill health
What We Offer

Southern Health NHS Foundation Trust offers a competitive salary range of £32,000 - £43,041 per annum, depending on experience, along with a comprehensive benefits package, including pension scheme, annual leave, and training opportunities.
