Job Summary

We are seeking a skilled and compassionate Care Assistant to join our team in providing end-of-life support to patients and their families in the comfort of their own homes.

As a Care Assistant, you will work closely with our multidisciplinary team to deliver high-quality care and support, ensuring the dignity and well-being of our patients at all times.

Key Responsibilities:

  • Deliver individualized personal care and support to patients, maintaining high standards of care and dignity.
  • Work collaboratively with patients, families, and healthcare professionals to develop and implement personalized care plans.
  • Monitor and report changes in patient symptoms, conditions, and circumstances to the registered health professional.
  • Support patients with physical and personal care needs, maintaining dignity and respect at all times.
  • Identify and act on any risk of safeguarding to patients and their families, in accordance with our policies and procedures.
  • Keep accurate and legible records, in accordance with legislative requirements and our practice and procedure.

  • Excellent communication and interpersonal skills.
  • A high level of empathy and understanding of the needs of patients and their families.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
