As a Casual Reablement Assistant, you will play a vital role in supporting individuals to maintain their independence and confidence, enabling them to live well with Dementia.

Key Responsibilities
  • Work collaboratively with individuals and their circle of support to develop and implement personalized support plans.
  • Provide practical support and guidance to help individuals maintain their daily living skills and wellbeing.
  • Contribute to the prevention of care needs from escalating and work to prevent avoidable admissions to hospital and care homes.
  • Participate in crisis support 24/7, 365, ensuring the safety and wellbeing of vulnerable adults living within Myos House.
Requirements

To be successful in this role, you will need to be solution-focused, have the ability to motivate others, and possess excellent decision-making and interpersonal skills.

You will also need to be flexible and able to work autonomously in complex situations, with a willingness to undertake additional duties as required.
