">The Role">We are currently seeking a Support Worker to join our team at Hawarden, a new Supported Living service opening in November 2023.
">As a Support Worker, you will play a key role in providing person-centered care and support to individuals who are moving into their first adult placement.
">You will be responsible for implementing individual outcome and wellbeing plans, promoting independence, managing risks, and encouraging relationships.
">Key responsibilities include:

- Managing risks and ensuring legal and best practice compliance">
- Facilitating effective communication and maintaining detailed records">
- Overseeing medication processes and supporting individuals in self-administration">
