As a Care Support Specialist, you will play a vital role in assisting individuals to live independently and achieve their personal goals. Your responsibilities will include:

  1. Providing personalised support to clients in their daily activities, including meal preparation, personal hygiene, and household tasks.
  2. Implementing and monitoring individual support plans, ensuring the well-being and safety of clients.
  3. Encouraging and assisting clients in engaging with their community and building social connections.
  4. Collaborating with healthcare professionals, family members, and other stakeholders to ensure comprehensive care.
  5. Documenting and reporting on client progress and any incidents or concerns.
  6. Participating in team meetings and training sessions to enhance skills and knowledge.
  7. Working flexibly across a 7-day rota to meet the needs of the people we support, including night shifts.
Requirements:

To be successful in this role, you will need:

  1. Previous experience in a similar role or a relevant qualification in health and social care.
  2. Strong interpersonal and communication skills.
  3. Empathy and a person-centred approach to care.
  4. Ability to work independently and as part of a team.
  5. Understanding and commitment to promoting independence and inclusion.
