Job Title: Primary Care Specialist

We are seeking an exceptional Primary Care Specialist to join our team at Wentworth 1 Primary Care Network. This is an exciting opportunity for a skilled professional to deliver high-quality patient care and contribute to the development of our network.

Job Description:
  • Undertake clinical decisions at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
  • Assess undifferentiated patients with a range of acute, non-urgent, and chronic medical conditions.
  • Undertake pre-booked emergency clinics, face-to-face consultations, telephone or video consultations.
  • Work as part of a larger primary care network to improve patient care and ensure more efficient ways of working.
  • Promote a learning environment for patients, nurses, and other health professionals.

About Us:
Wentworth 1 Primary Care Network includes 7 member practices, providing primary care for over 52,000 patients in the Rotherham area. Our aim is to develop a stronger, more resilient primary care offer for all our patients by working together to coordinate, plan, and share resources. We strive to design and deliver innovative services better placed to meet the needs of our communities, ensuring better health outcomes for patients and utilizing resources efficiently.

Required Skills and Qualifications:
  • Degree or postgraduate qualification in a relevant field.
  • Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's ePortfolio (supported) Route.
  • Experience of working in primary care.
  • Broad and sound knowledge of complex and common conditions in primary care in both adults and children.
  • Experience of telephone triage.

Estimated Salary: £45,000 - £60,000 per annum, dependent on experience.
