Diabetes Care Specialist

Connaught Resourcing is partnering with a national service provider to recruit permanent Diabetes Specialist Nurses for primary care roles across the country. Our client aims to develop a nationwide Diabetes team, considering nurses from all locations.
This hybrid-working position involves conducting clinics in community settings (e.g. GP practices, care homes, health centres) and working from home in between. As a proactive member of the team, you will identify sub-optimally controlled patients with Type 2 diabetes, optimize their oral therapy, conduct point-of-care testing, and refer for specialist insulin treatment when necessary, while exercising corporate nursing responsibilities.
Working Hours:
Monday-Friday 8:30am-5:30pm (four days part-time considered).
Essential Criteria:
- Diabetes diploma or equivalent
- A minimum of three years' experience (post-registration) managing diabetes within primary care
- Knowledgeable and confident in providing education, support, and advice to people with Type 2 Diabetes
- Skills and knowledge in optimizing oral hypoglycaemics
- Full driving license
- Current NMC registration
- IT literacy, including primary care systems

£45,000 annual salary + package
