Care Planning Specialist

We're seeking a Care Planning Specialist to join our dynamic team and help deliver the best outcomes for those we care for in the community and at home.

As a Care Planning Specialist, you will be responsible for assessing and writing person-centred care plans based on individual strengths, needs, and aspirations. You will also conduct risk assessments, auditing, and ensure compliance with CQC standards. Working with clients and their families, you will create tailored care packages that focus on long-term outcomes. Additionally, you will collaborate with a wide range of health professionals and services to deliver holistic support.

The ideal candidate will have a Level 2 qualification (or equivalent) in Health & Social Care, a full UK driving license, and access to your own vehicle. Strong English and computer skills are essential. A flexible and reliable attitude, with the ability to work independently and as part of a team, is also required.
