Mental Health Practitioner

We are seeking a skilled Mental Health Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Community Mental Health Service, you will work closely with patients, colleagues, and partner agencies to deliver high-quality care and support.
Your primary responsibility will be to provide evidence-based interventions and assessments for individuals presenting with mental health problems. You will work across a blended caseload of common mental health disorders and severe mental illness patients, including those with emotional needs.
As a role model, you will maintain the highest standards of professionalism and act as a positive influence on colleagues. You will also have the opportunity to design and deliver evidence-based recovery-focused mental health interventions and groups.
We offer a challenging and exciting opportunity to gain experience of working across North Community Mental Health Service, developing relationships with Primary Care, partner agencies, and wider mental health services.
Key Responsibilities:
- Provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for people presenting with mental health problems.
- Represent the team on specific projects and working groups as required.
- Work across all clinically appropriate patients, 17 years old and over, to deliver evidence-based interventions.
- Manage your own caseload and work collaboratively with colleagues to achieve high-quality patient outcomes.

Requirements:
- A relevant professional qualification relating to Health or Social Care, e.g., in social work, nursing, OT with current professional registration.
- Recent evidence of continuing professional development.
- Able to meet the travel requirements of the post.
