About the Role
We are seeking a highly skilled and compassionate Community Staff Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality patient care in the comfort of patients' own homes.
Key Responsibilities
- Undertake first visits and assessments to identify patients' needs and develop personalized care plans.
- Provide evidence-based care and support to patients with a range of health needs, including chronic conditions and complex care requirements.
- Work collaboratively with patients, families, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Participate in ongoing professional development and training to maintain expertise and stay up-to-date with best practices in community nursing.

Requirements
- Registered General Nurse (RGN) qualification and current NMC registration.
- Experience in community nursing, with a strong understanding of patient-centered care and evidence-based practice.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
- A full UK driving license and access to a vehicle for work purposes.
