About the Role

We are seeking a highly skilled and organized Care Coordinator to join our team at Ahava Care Group. As a Care Coordinator, you will play a vital role in supporting our GP Support Team in delivering high-quality patient care.

Key Responsibilities:

  • Support practices across the PCN with patient-related administrative requests
  • Identify patients who would benefit from proactive care and organize multidisciplinary meetings
  • Act as a champion for patient choice and empowerment, ensuring teams have access to tools for patient-centered care
  • Manage the call and recall system for GP targets and administer e-consultations
  • Liaise with PCN teams and build relationships with GPs

Requirements:

  • Excellent IT and communication skills
  • Natural ability to organize and coordinate multiple elements
  • Problem-solving skills and ability to take difficult situations in stride
  • Confidentiality and common sense approach
