We are seeking a skilled and dedicated Registered General Nurse to join our team as a Community Nurse Specialist. About the Role

This is a band 5 secondment vacancy for 12 months with the possibility of extending the contract to cover maternity leave the following year. As a Community Nurse Specialist, you will be responsible for providing evidence-based, high-quality nursing care to patients in their own homes. You will work closely with community nursing teams, carrying out nursing assessments, treatments, and care plans.

Key responsibilities include:

  • Providing holistic nursing care to patients in their own homes
  • Liaising with other healthcare professionals, such as GPs, consultants, and hospital nurses
  • Assessing patient needs and formulating care plans
