Job Overview:
Our team at Somerset NHS Foundation Trust is seeking an experienced Community Care Professional to join our community care team. As a key member of the team, you will be responsible for providing high-quality care to older adults in the Taunton, West Somerset & Exmoor National Park areas.

Job Description:
The successful candidate will work closely with community teams to provide effective multidisciplinary collaborative care and act as Responsible Clinician for community patients subject to a compulsory treatment order under the Mental Health Act (CTOs). You will also provide assessment, diagnosis and treatment of mental health needs of community patients in their community area.

Key Responsibilities:

What We Offer:
- Salary: £85,000 - £115,000 per annum depending on experience.

Qualifications and Experience:
- Significant experience of assessment and care of older adults.
- Experience of QI or service development with older adults.
- Under and postgraduate teaching experience.
