About the Role

We are seeking a highly motivated and experienced Social Worker to join our Adult Social Care Hub. As a key member of our team, you will play a vital role in supporting vulnerable adults to live fulfilling lives in their own communities.

Key Responsibilities
  • Manage a caseload of adult social care cases, prioritizing and organizing workload effectively with regular supervision and guidance.
  • Work collaboratively with multidisciplinary colleagues to ensure individuals receive the right care, at the right time, in the right place.
  • Take a strengths-based approach to support individuals to live their best lives, reducing dependence on long-term care.
  • Support the delivery and maintenance of high standards of performance across the service.
  • Work within allocated resources as set out in our scheme of delegation.
  • Take an evidence-based approach to practice.
  • Participate in regular supervision and appraisal, taking responsibility for your own learning and development.
