Nurse Required for Rehabilitation and Complex Care Planning

We are seeking a highly skilled and dedicated Registered Nurse to join our team at St David's Hospital, specialising in rehabilitation and complex care planning under the 'Discharge to Assess' model of care. This pioneering approach has been successfully implemented within NHS Wales, and we are looking for a nurse who can contribute to our efficient and effective multidisciplinary team.
Key Responsibilities- Deliver high standards of care to patients, prioritising their physical, psychological, social, and spiritual needs.
- Act as a patient advocate, safeguarding their interests and promoting a positive image of the UHB.
- Develop and implement individualised care plans, utilising the nursing process of assess, plan, implement, and evaluate.
- Ensure awareness of legal requirements regarding medication management, Deprivation of Liberty safeguards, Mental Capacity Act, and Mental Health Act.
- Recognise and respond to patient deterioration, completing observations and raising safeguarding concerns as necessary.
