Job Title: Adult Advanced Practitioner

We are seeking a highly skilled and experienced Adult Advanced Practitioner to join our team at University Hospitals of North Midlands NHS Trust. As a key member of our Urgent Care Response Team, you will play a vital role in delivering high-quality patient care within the community.

Key Responsibilities:
  1. Provide expert clinical care to patients in their own homes, utilizing advanced assessment and critical thinking skills to diagnose and manage complex health conditions.
  2. Work collaboratively with multidisciplinary teams to coordinate seamless care pathways and ensure effective communication with patients, families, and healthcare professionals.
  3. Prescribe and review medication, utilizing evidence-based practice and national protocols to ensure optimal patient outcomes.
  4. Assess and manage patients with mental health needs, referring to appropriate services as necessary.
  5. Lead and manage staff performance, training, and supervision, including annual appraisals and ensuring accurate and complete documentation.

Requirements:

  • Registered Healthcare Professional with an MSc in Advanced Clinical Practice or equivalent.
  • Independent Non-Medical Prescriber with expertise in acute care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Strong leadership and management skills, with experience in staff supervision and training.
