Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Hafod Nursing Home. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our residents, promoting their independence and dignity.

Key Responsibilities

  • Deliver exceptional nursing care to residents with complex needs
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with multidisciplinary teams to ensure seamless care delivery
  • Conduct regular assessments and evaluations to monitor resident progress
  • Provide education and support to residents and their families on health-related topics

Requirements

  • Current registration with the Nursing and Midwifery Council (NMC)
  • Proven experience in a nursing role, preferably in a care home setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong problem-solving and critical thinking skills
