Job Overview

The Cardiff and Vale University Health Board is seeking a highly motivated and enthusiastic individual to fill the role of a Rehabilitation Assistant in Stroke Care. As a member of our multidisciplinary team, you will play a vital part in delivering high-quality patient care and rehabilitation services to individuals recovering from stroke.

Key Responsibilities

This position involves supporting patients in their rehabilitation journey, working closely with healthcare professionals to develop personalized care plans, and promoting independence and well-being. You will also contribute to maintaining a safe and supportive environment for patients, families, and carers. Your duties may include rotating between the 38-bedded inpatient Stroke Rehabilitation Centre (SRC) and Early Supported Discharge (ESD) community stroke team, ensuring continuity of care and effective communication among team members.

Essential Qualifications and Skills

To be successful in this role, you must possess GCSE-level numeracy and literacy skills, as well as a recognized qualification in NVQ level 4 or equivalent. You should have experience working in health and social care settings, dealing with people individually or in groups, and demonstrating self-motivation and adaptability to change. Additionally, your excellent communication skills and ability to motivate others will enable you to effectively advocate for patients' rights and needs.
