About the Role
We are seeking a highly motivated and compassionate Support Worker to join our team at Priory Group. As a Support Worker, you will play a vital role in providing person-centered support and care to individuals with complex needs.
Key Responsibilities
- Ensure that individuals have their personal needs and requirements met, including personal care, participation in activities, and support with daily living tasks.
- Support individuals in achieving their outcomes in the way they choose.
- Complete daily administration, including updating person-centered support and care plans, updating charts, and completing accurate daily notes.
- Maintain professional relationships while developing a good rapport with individuals, their families, colleagues, visitors, and other stakeholders.
- Contribute to the maintenance of safety, security, and confidentiality standards through adherence to policies and procedures.
- Undertake keyworking responsibility for individual service users, supporting them to plan goals and personal outcomes and ensuring these are comprehensively documented and reviewed.
Requirements
- Basic numeracy skills
- Basic literacy skills
- Willingness to work towards a Diploma/SVQ 2 in Health and Social Care
- Diploma/S/NVQ2 in Health and Social Care (desirable)
- Knowledge and understanding of the needs of individuals with learning disabilities, autism, dementia, Prader-Willi Syndrome, or mental health conditions (desirable)
