Community Support Practitioner Senior

We are seeking a highly skilled Community Support Practitioner Senior to join our team. This is an exciting opportunity for an experienced professional to lead our Community Accommodation Support service in Grimsby/Hull.
The successful candidate will have a strong understanding of community support principles, with experience of working with vulnerable adults. You will have excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders.
Your key responsibilities will include:
- Providing direct support to service users, including assessment, planning, and review.
- Working closely with area operational and central support teams and liaising with a range of external agencies and partners.
- Contributing to the development and implementation of service improvement plans for a service or group of services.
