Job Description
As a Healthcare Support Worker in our End of Life Care team, you will play a vital role in providing person-centered care to patients and their families. You will work collaboratively with multidisciplinary teams to assess, plan, and deliver care that meets individual needs.

Key Responsibilities
• Conduct comprehensive assessments of patients' physical, emotional, and social needs
• Develop and implement individualized care plans in collaboration with healthcare teams
• Deliver high-quality care, including personal care, medication management, and emotional support
• Communicate effectively with patients, families, and healthcare professionals

Requirements
• NVQ level 3 health and social care qualification or equivalent experience
• Proven experience working in a healthcare setting, preferably in end-of-life care
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
