We are seeking a highly skilled and innovative Integrated Cluster Network Manager to lead and manage a team of multi-agency professionals, focusing on the needs of adults with physical frailty, chronic illness, and primary mental health needs.
Key Responsibilities- Operate and strategically develop the Integrated Team, responsible to both the Local Authority and University Health Board.
- Manage services across agencies, working to develop seamless pathways of care and support for adults with complex needs.
- Bring a multi-disciplinary approach to provide timely professional support for people's needs, ensuring choice and control for individuals and placing the person at the centre of their care.
- Lead staff in providing an effective team around the person, enabling them to support the individual and those important to them through holistic person-centred practice.
- Undertake a key role in the development of the Transformation agenda and Accelerated Cluster working in the East Cluster network area, working closely with the Cluster Partnership and Primary Care services.
- Experience of multi-disciplinary and multi-agency working, preferably across traditional organisational boundaries.
- The ability to greet customers through the medium of Welsh.
- A criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
- The council's Hybrid Working Policy applies to this post, providing a framework for establishing how you will undertake working hours between your home and the office.
