Patient Advocacy Coordinator

This is an exciting opportunity to work in a dynamic healthcare environment, with a competitive salary range of £28,000 - £32,000 per annum.

• Cover calls on an electronic rota system and ensure weekly rotas are completed on time.
• Evaluate initial contact between care workers and service users to assess care plans and ongoing needs.
• Develop relationships with service users and care staff through regular telephone assessments, monitoring satisfaction and care worker performance.

The successful candidate will possess excellent communication skills, enabling them to liaise with professionals, clients, and management to improve care standards.

A strong focus on maintaining high-quality person-centered services is essential in this role, adhering to national minimum standards.
