**About the Opportunity**
NHS Dumfries & Galloway is undergoing a transformative period to develop multi-disciplinary integrated teams. Our Home Teams will deliver person-centred care, prioritising independence as the core of service delivery.
We are seeking enthusiastic registered Occupational Therapists with a keen interest in working within adult community health settings. This role focuses on providing occupational therapy assessment and interventions to maximise client function and independence, primarily through short-term rehabilitation and proactive, preventative measures.
You will be responsible for managing a caseload to provide a full service of assessment, appropriate management, onward referral, and discharge to a range of patients referred to the service. Collaboration with multi-professional teams ensures coordinated care for patients.
The focus lies in utilising professional skills to deliver rehabilitation and enablement, promoting positive outcomes for individuals.
