Job Title: Care Coordinator

We are seeking a compassionate and organized individual to work with older people in residential care, providing high-quality support and care that prioritizes their needs and well-being.

In this role, you will be responsible for building strong relationships with service users, families, and healthcare professionals, developing and maintaining effective care plans, and providing emotional support and encouragement to residents. You will also work collaboratively with colleagues to achieve excellent standards of care and maintain accurate records and communicate resident needs to the Line Manager.

The ideal candidate will have previous experience as a Senior Support Worker or similar role, excellent communication and interpersonal skills, and the ability to work independently and as part of a team. A good understanding of documentation is also essential.
