Job Title: Adult Advanced Practitioner

Saxmundham Health is seeking a highly skilled and experienced Adult Advanced Practitioner to join our team.

Key Responsibilities:
  • Critical Thinking and Decision Making: Demonstrate a critical understanding of complex situations and make informed decisions that prioritize patient care and safety.
  • Leadership and Management: Lead and manage a clinical team, providing guidance and mentorship to ensure high-quality patient care.
  • Communication and Collaboration: Develop and maintain effective relationships with patients, families, carers, and healthcare professionals to ensure seamless care coordination.
  • Clinical Expertise: Provide expert clinical advice and guidance to patients and healthcare professionals, ensuring the highest standards of care.
  • Professional Development: Continuously assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development.
  • Service Improvement: Actively seek feedback and involvement from individuals, families, carers, communities, and colleagues in the co-production of service improvements.
  • Medicine Management: Maintain accurate clinical records, ensuring SNOMED CT codes are used effectively, and monitor the safe storage, rotation, and disposal of medicines.
  • Safeguarding: Support the team with all safeguarding matters in accordance with local and national policies.
  • Health Promotion: Deliver opportunistic health promotion where appropriate, and participate in local initiatives to enhance service delivery and patient care.
  • Continuous Improvement: Continually review clinical practices, responding to national policies and initiatives where appropriate, and participate in the review of significant and near-miss events.
