About the Role

We are seeking a highly skilled and compassionate CYP Family Keyworker to join our team at HCRG Care Group. As a key member of our Child and Family wellbeing services, you will play a vital role in delivering high-quality support to children and young people with complex needs.

Key Responsibilities
  • Accept, allocate, and assess cases, working closely with the Case Load Manager to identify at-risk children/young people and their families from the Dynamic Support Register (DSR).
  • Build and maintain trusting relationships with children, young people, and their families, ensuring consistent allocation of cases.
  • Lead and participate in the out-of-hours rota, providing clinical and caseload supervision.
  • Liaise with health and social care, education, and police colleagues to improve access, continuity of care, and well-planned transitions.

Person Specification

We are looking for an individual with experience in Autism, Learning Disabilities, Co-production & Trauma, although we will review all applications from individuals looking to gain exposure in these areas. The ideal candidate will have excellent time management skills, the ability to organize a high case load, and be compassionate, resilient, and dedicated to changing the lives of the families that HCRG Care Group strive to make a difference to.

Having access to a vehicle and driving license is essential for this role.
