About this Opportunity
We are seeking a highly skilled and motivated Healthcare Professional to join our Rapid Response & Intermediate Care team. As a key member of our team, you will provide expert interventions and rehabilitation to patients in their own homes, promoting independence and preventing hospital admission.
Main Responsibilities
- Provide comprehensive specialist assessments, reviews, and case management for patients and carers.
- Develop and implement personalized care plans to promote health gain and maximize independence.
- Collaborate with a range of professionals to ensure timely and effective delivery of care.
- Lead and manage the Rapid Response & Intermediate Care team, allocating workloads and supporting staff development.
Requirements
- Registered Allied Healthcare Professional with HCPC registration.
- Degree or Diploma in Physiotherapy or Occupational Therapy.
- Experience of recent Continuous Professional Development.
