**Overview of the Role**

We are seeking a qualified Adult Social Worker to join our Older Person team in West Yorkshire. The successful candidate will be responsible for providing high-quality care and support services to adults, with a focus on enhancing their overall well-being and quality of life.

About the Position

The Adult Social Worker will work closely with our clients, their families, and other healthcare professionals to ensure seamless care coordination. You will assess client needs, develop and implement individualized care plans, and coordinate with healthcare professionals and family members to ensure effective care delivery.

Responsibilities

  • You will provide information, advice, and guidance on social care options, helping individuals to make informed decisions about their care.
  • You will work closely with our clients, their families, and other healthcare professionals to ensure seamless care coordination.
  • You will also provide education and training to clients and their families on topics such as health promotion, disease prevention, and wellness strategies.

Requirements

  • You must possess a degree in Social Work or a related field, as well as relevant experience working with vulnerable adults.
  • Excellent communication and interpersonal skills are essential, with the ability to build strong relationships with clients, colleagues, and partners.
  • You should be highly organized, with the ability to prioritize tasks, manage competing demands, and meet deadlines.
